Concurrent treatment with another provider: If you currently see a mental health professional or another type of provider that could intersect or overlap with our work, I ask that you discuss this engagement with them in advance to deconflict goals and approaches and ensure that coaching is a good idea for you currently. I may ask for consent to speak to that provider to coordinate our efforts*. If it becomes evident that the approaches conflict or cause more harm than good, Dr. Pearl Zurich & Associates, LLC has the right to terminate our work based on our professional opinion and reimburse you for the remainder of your sessions.***
Confidentiality & Privacy: Your information will be kept private and confidential via Simple Practice, a secure HIPAA-compliant telehealth platform. Client communications will only be released with your written permission, or if I am legally obligated to breach confidentiality for one of the following reasons:
- If I assess that you may be at risk of danger to yourself or someone else
- If a third party has funded your coaching and requires us to report certain information.
Mutual Nondisclosure: I ask that you do not disclose information, materials, intellectual property, or fees related to any services provided by Dr. Pearl Zurich & Associates, LLC to any third parties.
